DOT Medical Examiner

My name is Dr. Randolph Rosarion  M.D.  I a Board Certified physician and I will be performing your examination. I am also certified by the National Academy of DOT Medical Examiners and a Certified Medical Review Officer. See About Us  page for more information.

Providing that no complicated medical issues arise during the course of your evaluation, it can take about half an hour. 

Although no special preparations are required, do maintain a healthy lifestyle.  Proper diet and exercise, and avoiding tobacco use will help you achieve a more ideal body weight and blood pressure.  

If you wear corrective lenses (eyeglasses or contact lenses) bring them with you for the examination.  If you recently had an eye exam performed by an opthalmologist or optometrist bring the exam results with you.  Also, if you use a hearing aid bring them along as well for the examination.

Of course, do not forget to bring your driver's license.

First of all relax.  If this is a new certification we will do our best to help you with any questions and concerns that you may have.  Be as thourough as you can in filling out your medical questionaire.  Explain any yes answers in detail and list any medications that you are taking along with the names of your treating physicians. Sign your history form.

No.  The medical form, Form 649-F( 6045) Medical Examination Report For Commercial Driver Fitness Determination will be provided by our office.  If you like, you can download the form to fill out before your appointment from our web page or from the link provided to the FMCSA (Federal Motor Carrier Safety Administration) website.  Our office will also provide the Medical Examiner's Certificate to our qualified drivers after the examination.

When requested for example by your employer, a Urine Drug Screen and Alcohol Breath Test may be performed for an aditional charge. 

We perform the required urine dipstick test to evaluate for protein, blood, glucose (sugar), and specific gravity (concentration of particles in urine).  It helps screen for disease such as diabetes, kidney problems, etc. 

Yes.  Qualified Drivers are issued their Medical Certificate the same day.
